Apologies for the delay. Our vendor was at CES and just got back to us with the documents that we needed. Please see the below set of instructions.
1. Go see an audiologist to have your ear impressions created. Have them follow the guidelines on this document: http://dro.ps/audiologist_instructions
2. Fill out this form with your order details: http://dro.ps/ctm_order_form
3. Ship both your impressions AND order form to:
Clear Tune Monitors

**Please use a ship method that comes with a tracking number, for CTM’s reference
4. It will then take 4-5 weeks for production
5. Once the CTM IEMs are completed, CTM will ship the finished IEM to you directly.
